Nano-silver wound dressing

A nano-silver wound dressing consists of a skin contact layer made from hydrophilic cloth and directly contacting wound on surface of skin, a disinfecting (or bactericidal) antitoxic layer made from activated charcoal cloth impregnated with nanocrystalline silver, a blood absorbing and styptic layer made from non-woven cloth with superabsorbent polymer, an isolation layer made from composite fabric with pore size less than 5 .mu.m, and an elastic bandage for fixing a main body on the site of wound. Edges of the isolation layer and the skin contact layer are integrated to form a main body while the disinfecting (or bactericidal) antitoxic layer as well as the blood absorbing and styptic layer are separated from each other and both enclosed inside the main body.

TPU Development to Treat Diabetic Foot Successfully Used in Mexico

Scientists from Tomsk Polytechnic University along with National Autonomous Mexico University develop techniques to treat diabetic foot syndrome with silver nano-particles which special insoles are treated with. The techniques help to fight ulcers appearing on feet in diabetic patients, facilitates their healing and disinfection, reducing the risk of amputation.

Diabetic foot syndrome is one of the latest and most serious complications of diabetes. Due to the large amount of sugar in the body there are changes in peripheral nerves, blood vessels, skin and soft tissues, bones and joints of the patient. Infections, ulcers, suppurations and so on are emerging. Up to 15% of people with diabetes have the risk of developing ulcers on feet. In the advanced form diabetic foot syndrome can lead to amputation.

Silver preparations being developed by Tomsk Polytechnic University jointly with Novosibirsk and Mexican counterparts are able to reduce such risks.

The research has shown silver’s antibacterial properties facilitate rapid healing of ulcers and suppurations in patients with diabetic foot syndrome. Together with colleagues from Mexico, where the problem is particularly acute, we are working to create special insoles for diabetic patients.

The development has passed clinical tests. In patients who had used the insoles impregnated with silver nanoparticles, leg ulcers healed up, the risk of amputations significantly reduced:

The present study selected the predominant multi antibiotic-resistant diabetic foot ulcer (DFU) derived bacterial isolates such as Pseudomonas aeruginosa (PA), Escherichia coli (EC), Staphylococcus aureus (SA) and Bacillus subtilis (BS) and evaluated their response against the well-characterized Aerva lanata (AL) reduced multiple phytochemicals fabricated silver nanoparticles (AL-AgNPs). The overnight culture of DFU isolates was processed and subjected to various studies such as antimicrobial activity, growth kinetics, biofilm disruption, reactive oxygen species (ROS), membrane leakage, membrane permeability, and damage and genotoxicity. The molecular docking of AL phytochemicals was also performed with bacterial enzyme DNA gyrase. Interestingly, AL-AgNPs were produced the remarkable antibacterial effect against the resistant DFU isolates, which was closely similar to the effect of AL-AgNPs observed against the reference strains. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of AL-AgNPs against the DFU isolates were found to be 5–15 μg/mL and 10–20 μg/mL, respectively. The AL-AgNPs were depicted a concentration-dependent growth inhibition of DFU bacterial isolates. The MIC and MBC of AL-AgNPs were effectively destroyed the preformed biofilms of DFU isolates. Furthermore, the MBC of AL-AgNPs was displayed the increased intracellular ROS accumulation, membrane leakage, permeability and damage, and genotoxicity in the DFU isolates. Additionally, the in silico study revealed that the AL phytochemicals were fitted over the binding pocket of the DNA gyrase B subunit. The observed results were confirmed that the negative impacts of the AL-AgNPs at the level of the membrane and intracellular components of DFU isolates.

Type 2 diabetes (T2D) in developed countries have a prevalence of 11% with diabetic foot infections as the most common cause of hospitalization and amputation. To achieve healing of the diabetic foot ulcer wounds, appropriate dressings are essential and their effectiveness can be enhanced with nanoparticles, nevertheless ideal combinations of dressing composition and nanodrugs require further testing in humans. We have developed a calcium alginate dressings with ZnO nanoparticles (CAZnODs) for the treatment of diabetic foot ulcers in human patients. To test the efficacy of CAZnODs we designed a randomized controlled clinical experiment on 26 T2D patients with foot ulcers. The patients were randomized into two groups: G1 treatment with calcium alginate with NPs (G1; n = 16), and group 2 received the treatment without NPs (G2, n = 10). The bandage change was performed every 48 h The duration of the protocol was established at 10 weeks. Here, we report healing was achieved in patients, with 75% wound closure in G1 under treatment with NPs of calcium alginate versus 71% in G2 (calcium alginate without NPs) (p = 0.011). The average healing time was 48 days in G1 and 72 days in G2. Our data shows that CAZnODs were well tolerated and did not interfere with the wound healing process. The final wound area and time of healing support the hypothesis that the use of calcium alginate dressings with nanoparticles may induce better tissue regeneration while avoiding T2D complications such as secondary infections.

Diabetic foot ulcer (DFU) is one of the most serious complications of diabetes, potentially resulting in wound infection and amputation under severe circumstances. Oxidative stress and dysbiosis are the primary factors that delay wound healing, posing challenges to effective treatment. Unfortunately, conventional approaches in these aspects have proven satisfactory in achieving curative outcomes. Recent research has increasingly focused on using nanoparticles, leveraging their potential in wound dressing and medication delivery. Their unique physical properties further enhance their therapeutic effectiveness. Among these nanoparticles, cerium oxide nanoparticles (CONPs) have garnered attention due to their notable beneficial effects on oxidative stress and microbial abundance, thus representing a promising therapeutic avenue for DFU. This review comprehensively assesses recent studies on CONPs in treating DFU. Furthermore, we elaborate on the wound healing process, ceria synthesis, and incorporating CONPs with other materials. Crucially, a thorough evaluation of CONPs' toxicity as a novel metallic nanomaterial for therapeutic use must precede their formal clinical application. Additionally, we identify the current challenges CONPs encounter and propose future directions for their development.

Diabetic foot ulcers (DFUs) are considered one of the most severe chronic complications of diabetes and can lead to amputation in severe cases. In addition, bacterial infections in diabetic chronic wounds aggravate this scenario by threatening human health. Wound dressings made of polymer matrices with embedded metal nanoparticles can inhibit microorganism growth and promote wound healing, although the current clinical treatments for diabetic chronic wounds remain unsatisfactory. In this view, this research reports the synthesis and characterization of innovative hybrid hydrogels made of carboxymethyl cellulose (CMC) and poly(vinyl alcohol) (PVA) chemically crosslinked by citric acid (CA) functionalized with silver nanoparticles (AgNPs) generated in situ using an eco-friendly aqueous process. The results assessed through comprehensive in vitro and in vivo assays demonstrated that these hybrid polymer hydrogels functionalized with AgNPs possess physicochemical properties, cytocompatibility, hemocompatibility, bioadhesion, antibacterial activity, and biocompatibility suitable for wound dressings to support chronic wound healing process as well as preventing and treating bacterial infections. Hence, it can be envisioned that, with further research and development, these polymer-based hybrid nanoplatforms hold great potential as an important tool for creating a new generation of smart dressings for treating chronic diabetic wounds and opportunistic bacterial infections.

Diabetes’s effects on wound healing present a major treatment challenge and increase the risk of amputation. When traditional therapies fail, new approaches must be investigated. With their submicron size and improved cellular internalisation, nanoparticles present a viable way to improve diabetic wound healing. They are attractive options because of their innate antibacterial qualities, biocompatibility, and biodegradability. Nanoparticles loaded with organic or inorganic compounds, or embedded in biomimetic matrices such as hydrogels, chitosan, and hyaluronic acid, exhibit excellent anti-inflammatory, antibacterial, and antioxidant properties. Drug delivery systems (DDSs)—more precisely, nanodrug delivery systems (NDDSs)—use the advantages of nanotechnology to get around some of the drawbacks of traditional DDSs. Recent developments show how expertly designed nanocarriers can carry a variety of chemicals, transforming the treatment of diabetic wounds. Biomaterials that deliver customised medications to the wound microenvironment demonstrate potential. Delivery techniques for nanomedicines become more potent than ever, overcoming conventional constraints. Therapeutics for diabetes-induced non-healing wounds are entering a revolutionary era thanks to precisely calibrated nanocarriers that effectively distribute chemicals. This review highlights the therapeutic potential of nanoparticles and outlines the multifunctional nanoparticles of the future that will be used for complete wound healing in diabetics. The investigation of novel nanodrug delivery systems has the potential to revolutionise diabetic wound therapy and provide hope for more efficient and focused therapeutic approaches.

Introduction To assess the efficacy of a chitosan-based gel (ChitoCare) for the treatment of non-healing diabetic foot ulcers (DFUs).
Research design and methods Forty-two patients with chronic DFUs were randomized to the ChitoCare or placebo gel for a 10-week treatment period and 4-week follow-up. The primary study end point was the rate of complete wound closure at week 10, presented as relative rate.

Results Thirty patients completed the 10-week treatment and 28 completed the 4-week follow-up. The ChitoCare arm achieved 16.7% complete wound closure at week 10 vs 4.2% in the placebo arm (p=0.297), 92.0% vs 37.0% median relative reduction in wound surface area from baseline at week 10 (p=0.008), and 4.62-fold higher likelihood of achieving 75% wound closure at week 10 (p=0.012). Based on the results of the Bates-Jensen Wound Assessment Tool, the wound state at week 10 and the relative improvement from the baseline were significantly better (median 20 vs 24 points, p=0.018, and median 29.8% vs 3.6%, p=0.010, respectively).

Conclusions ChitoCare gel increased the rate of the DFU healing process. Several secondary end points significantly favored ChitoCare gel.

Potential of hybrid lecithin-chitosan kaempferol ethosomes for the treatment of diabetic foot ulcer: In vitro and In vivo investigation

Hybrid lecithin-chitosan (LC) ethosomes were developed for maximizing encapsulation efficiency and release of Kaempferol (KMP) in controlled effect for topical delivery. The hybrid lecithin–chitosan offers the benefits of both components: enhanced lipophilicity and mucoadhesive, respectively. In this work, hybrid lecithin-chitosan KMP ethosomes (Eth) were prepared via a cold technique and assesses its physicochemical properties, in vitro and in vivo studies. The synthesized LC-Eth-vesicles of KMP reported vesicle size of 186.8 nm with polydispersity index of 0.285 and 31.9 mV of zeta-potential. The encapsulation efficiency was 96.2 %. In vitro drug release profile has shown prolonged release of KMP in a controlled manner over a period of 24 h. Around 80.3 % KMP was released from LC-KMP-Eth gel while only 46.9 % of KMP was released from the plain KMP-gel in 24 h. The release kinetic investigation the release of KMP followed the “Korsmeyer-Peppas model” with Fickian-diffusion mechanism. The LC-KMP-Eth has shown increased antimicrobial activity as compared plain KMP gel against multiple strains of MRSA. The histopathological analysis of skin samples revealed an increased re-epithelization with reduced wound ulcer which was also observed by the visual examination of the skin conditions. The in vivo results on Wistar albino rat models demonstrated superior wound closure efficacy of LC-KMP-Eth gel as compared to plain KMP gel. Around 2-fold overall increased pharmacokinetic parameters of KMP from LC-KMP-Eth gel after topical application, displayed a higher concentration of KMP in rat plasma, reveling a potent effect at the wound site and the gel also provide the moist environment and influencing cell adhesion, thus stimulating restorative action in diabetic foot ulcers.
