Sesamoid View X-ray vs Weightbearing Computed Tomography in the Measurement of Metatarsal Pronation Angle

Introduction/Purpose:
First metatarsal (M1) axial rotation as measured by the metatarsal pronation angle (MPA) has increasingly become recognized as a clinically relevant component of hallux valgus deformity and methods to realign the M1 in 3 dimensions have been developed. Preoperative imaging with weightbearing computed tomography (WBCT) is considered the best way to image and measure the MPA, however WBCT is not available to many foot and ankle surgeons. The aim of this project is to investigate if the MPA measured on sesamoid view weight bearing radiographs is similar to that measured on WBCT.
Methods:
A retrospective analysis of patient imaging studies was performed. We identified 25 feet with both sesamoid WBR and WBCT images from a consecutive patient database. Included subjects had no trauma or foot and ankle operation between acquisition of the two scans. MPA of the WBCT for each patient was initially measured. After an appropriate washout period, the observer then measured MPA of the WBR for the cohort. Demographic statistics were calculated, a paired t-test was performed to evaluate for any significant difference in angular measurement between the two scans.
Results:
Patient characteristics: mean age 53 +- 16 years, mean BMI 32 +- 8 kg/m2, 13 right and 12 left feet, 19 females and 6 males. Average time between sesamoid view WBR and WBCT was 5.7 +- 9.5 months. Mean difference in MPA between WBR and WBCT = 0.6 +- 3.9 degrees (p= 0.444, t=0.777).
Conclusion:
Among the cohort assessed, we found no significant difference present in the measurement of the metatarsal pronation angle as evaluated on sesamoid view weightbearing radiography compared with weightbearing computed tomography (Figure 1). Routine acquisition of a sesamoid view weightbearing radiograph may be considered as an appropriate substitute for weightbearing computed tomography in the preoperative evaluation of M1 rotation in the coronal plane. These findings may benefit physicians who do not have access to weightbearing computed tomography in their clinics.

Comparison of fore- and midfoot angles using 3DCT in standard weightbearing and sesamoid view position in feet with hallux valgus

Aims: To evaluate how fore- and midfoot coronal plane alignment differs in feet with hallux valgus (HV), using 3DCT when measured in standard weightbearing (SWB) versus sesamoid view (SV) position, and to determine whether first metatarsophalangeal (MTP) dorsiflexion affects the relationship between the first metatarsal (M1) head and the sesamoid bones.

Methods: A consecutive series of 34 feet that underwent 3DCT in SWB and SV positions for symptomatic HV was assessed, of which four feet were excluded for distorted or incomplete images. Two foot and ankle clinicians independently digitized a series of points, and measured a series of angles according to a pre-defined protocol. Measurements include navicular pronation angle, M1 head (Saltzman angle), and metatarsosesamoid rotation angle (MSRA).

Results: The mean age of the 30 patients was 57.5 years (SD 13.4). The mean navicular pronation angle was significantly smaller in the SV position (9.6° (SD 4.4°)) compared to the SWB position (16.4° (SD 5.8°); p < 0.001). There was a difference in MSRA between the SWB and SV positions, revealing an increase in MSRA in 22 patients, while there was a decrease in eight patients. In patients where the MSRA increased, the mean Saltzman angle was 2.5° (SD 5.7°) lower in the SV position versus the SWB position, while in patients where MSRA decreased, the mean Saltzman angle was 3.4° (SD 3.6°) greater in the SV position versus the SWB position.

Conclusion: MTP dorsiflexion causes supination of the navicular, while other first ray parameters remain unchanged, and has a greater influence on the M1 head coronal alignment than on the sesamoids. MTP dorsiflexion induces axial rotations of M1, which vary in direction and magnitude from one patient to another.

Impact of gout flare on health-related quality of life: a multi-center cross-sectional study in Thailand

Objective: Although the 5-level EQ-5D version (EQ-5D-5L) instrument has been used to determine health-related quality of life and health utility in gout, it is used in comparing health utility among gout flare (GF) and non-gout flare (non-GF) patients is still limited. This study aimed to compare health utility among GF and non-GF patients in Thailand.

Methods: In this multi-center cross-sectional study, patients with GF and non-GF were interviewed for the EQ-5D5L and EQ-Visual Analog Scale (VAS) instruments by rheumatologists or trained research staffs. Patients with GF were subdivided into 2 subgroups (those who received no treatment and those who received treatment less than 48 h after GF episode).

Results: Two hundred and sixteen patients (108 GF and 108 non-GF patients), males in 90.28%, were included. The gout disease duration was significantly longer in the non-GF than in the GF groups (median, 10 vs 5 years; p = 0.004). There was no difference in the tophi present between the two groups. When compared with the non-GF group, the GF patients significantly had low health utility (0.34 ± 0.36 vs. 0.89 ± 0.15, p < 0.001) and EQ-VAS score (54.73 ± 25.14 vs. 84.06 ± 13.38, p < 0.001). In the subgroup analysis of the non-GF group, there was insignificant health utility and EQ-VAS score between those with tophi and those without tophi (0.87 ± 0.14 vs. 0.90 ± 0.15, p = 0.124 and 83.36 ± 14.92 vs. 84.33 ± 12.83, p = 0.938, respectively).

Conclusion: This study found that GF clearly had a substantial impact on patients' quality of life. Targeted interventions in managing GF patients to improve their health outcomes are needed. Key Points • Patients with gout flare had lower health utility than those without gout flare. • The clinical significance of the utility and EQ-VAS was evaluated by the EQ-5D-5L instrument between gout flare and non-gout flare groups in Thai gouty patients. • Regarding the presence of tophi or disease duration, no significant differences in health utility and EQ-VAS were observed in the gout flare or non-gout flare group. • Targeted interventions for management of gout flare are needed to improve the health outcomes of gout flare patients.
