Treatment of Idiopathic Congenital Clubfoot in a Public Health Service: Comparison Between Ankle-Foot Orthosis and Abduction Orthosis

Background: To evaluate the efficacy of the Ponseti method in the treatment of idiopathic congenital clubfoot (CCF) in a public health service, during 2 distinct periods, focusing on the impact of orthosis type (ankle-foot orthosis and abduction orthosis).

Methods: This retrospective longitudinal cohort study analyzed medical records of 552 patients (842 feet) with CCF treated with the Ponseti method at a single hospital. Individuals were categorized into 2 groups: group 1 (2009 to 2017), which used an ankle-foot orthosis (AFO) after casts; and group 2 (2017 to 2021), which used an abduction orthosis (AO). Outcomes evaluated included relapse rates, residual deformities, and functional results using the modified Laaveg-Ponseti scale. The χ2 tests and Student and Welch t tests were used to assess statistical significance.

Results: The mean follow-up period was 10.3 years for group 1 and 5.3 years for group 2. Significant differences were observed between the groups (P<0.05). Group 1 (AFO) showed higher rates of relapse (71.9% vs. 31.2%) and residual deformities (58.8% vs. 32.6%) compared with group 2 (AO). Functional outcomes were also superior in group 2, with 81.8% achieving good/excellent results on the modified Laaveg-Ponseti scale, compared with 69.3% in group 1. Family adherence was lower in group 2 (16.5% nonadherence) compared with group 1 (10.5% nonadherence) and was related to higher relapse, yet overall outcomes were better.

Conclusion: The Ponseti method is effective for treating CCF, with superior results when an abduction orthosis is used. This highlights the importance of appropriate orthosis selection and continuous family support in public health services.

Clinical and Ultrasonographic Outcomes of Percutaneous Achilles Tenotomy Using an 18-Gauge Needle in Idiopathic Clubfoot: A Prospective Study

Introduction: Percutaneous Achilles tenotomy is a critical step in the Ponseti method for correcting residual equinus deformity in idiopathic clubfoot. Needle tenotomy has emerged as a minimally invasive alternative to conventional scalpel-based tenotomy, offering potential advantages in safety, procedural cost, and ease of performance. Prospective combined clinical and ultrasonographic evidence evaluating this technique from Indian centers remains limited.

Objective: The aim of the study was to evaluate the clinical and ultrasonographic outcomes of percutaneous Achilles tenotomy using an 18-gauge needle in children with idiopathic clubfoot managed by the Ponseti method.

Materials and methods: This prospective study included 45 children (76 ft) with idiopathic congenital talipes equinovarus. Serial Ponseti casting was performed until residual equinus was the only deformity remaining, after which percutaneous Achilles tenotomy was performed using an 18-gauge needle under general anesthesia. Patients were evaluated preoperatively and at 3 weeks, 2 months, 6 months, and 1 year using the Pirani score, passive dorsiflexion angle at the ankle, and foot bimalleolar angle on podogram. Ultrasonography was performed at the 3-week follow-up to assess tendoachilles healing.

Results: The mean age at tenotomy was 7.3 ± 2.76 months, with male predominance (71.11%) and bilateral involvement in 68.89% of cases. On ultrasonography at 3 weeks, 14 of 76 ft (18.42%) showed a tendon defect, and 9 of 76 ft (11.84%) showed peri-tendinous collections; both abnormalities resolved by the 2-month follow-up. The mean Pirani score improved significantly from 2.11 ± 0.40 preoperatively to 0 at 1 year (t = 42.93, df = 66, P < 0.05). The mean passive dorsiflexion angle improved from 128.74° ± 12.04° to 76.21° ± 2.15° (mean difference 52.53° ± 11.57°; t = 36.90, df = 65, P < 0.05), and the mean foot bimalleolar angle improved from 70.09° ± 2.70° to 81.74° ± 1.58° (t = -48.96, df = 65, P < 0.05). Forty patients (88.90%) maintained correction without additional surgery; four patients (8.88%) required posteromedial soft-tissue release, and one patient (2.22%) required unilateral tendoachilles lengthening.

Conclusion: Percutaneous Achilles tenotomy using an 18-gauge needle is a safe, effective, and minimally invasive technique for correcting residual equinus in idiopathic clubfoot. The procedure yields favorable clinical correction, satisfactory ultrasonographic tendon healing, and a low rate of secondary surgical intervention.

Congenital talipes equinovarus (clubfoot) stays as one of the main reasons of preventable childhood infirmity in many low- and middle-income countries, it is because of delayed diagnosis, poor care pathways, and poor long-term treatment adherence. The Ponseti method is widely known as the gold standard for non-surgical treatment; its success significantly relays on the sustained caregiver engagement and continuous brace rules compliance, which stay challenging in resource-constrained and rural settings. In response to these research gaps, this study proposes an integrated framework that combines artificial intelligence-enabled digital health technologies with co-production principles to strengthen treatment adherence in clubfoot management. Drawing on the concept of co-production, patient engagement, and human–AI collaboration in healthcare, the current study develops a multistakeholder conceptual model where caregivers, clinicians, and practitioners collaboratively design and implement AI-supported treatment pathways. The framework conceptualizes the role of intelligent mobile health systems in facilitating early detection, personalized educational guidance, real-time adherence monitoring, and constant feedback between families and healthcare contributors. By embedding AI tools within a co-produced care ecosystem, the model aims to improve trust, accountability, and shared responsibility through the treatment duration. This paper contributes to the literature by advancing a theoretically grounded and practice-oriented model that integrates digital innovation with participatory healthcare delivery. The proposed framework offers a scalable approach for improving adherence-driven outcomes in clubfoot treatment and provides conceptual guidance for the design of AI-enabled co-production tactics in pediatric orthopedics and other chronic childhood circumstances. The paper will suggest suitable research designs for future work.

Background: Early-presenting Congenital Talipes Equinovarus (CTEV) is usu-
ally corrected non-operatively with the Ponseti method, achieving excellent out-
comes. Neglected CTEV (NCTEV), however, is challenging and often requires
surgery. Objectives: These are to: 1) describe clinical profiles, reasons for ne-
glect, and treatments among NCTEV presenting after walking age; 2) assess
functional satisfaction, anatomical outcomes, and quality of life after treat-
ment; 3) examine effects of age at presentation, deformity rigidity, and anaes-
thesia choice on outcomes; and 4) document common surgical complications.
Methods: A retrospective descriptive cross-sectional review was conducted on
NCTEV cases treated surgically at Usmanu Danfodiyo University Teaching Hos-
pital, Sokoto (July 2018-June 2025). Patients included in the study were aged
≥2 years, had primary NCTEV with surgical treatment, complete records, and
≥9-month follow-up. Exclusion criteria were incomplete data, secondary club-
foot, and non-surgical care. Data (demographics, causes of neglect, Pirani score,
prior care, operative details, post-operative care, complications, outcomes like
plantigrade foot, ambulation, and cosmesis) were extracted using a structured
form and analysed in SPSS version 25 (p < 0.05). Confidentiality was main-
tained; consent was waived per institutional ethics for retrospective studies.
Results: Of 126 surgical clubfoot patients, 58 (46%) met criteria, aged 3 - 23 years
(mean = 13.9). Females were 65.5% (F:M = 1.9:1). A total of 103 feet were af-
fected: 45 bilateral, 9 left, and 4 right. All presented with poor Pirani score (≥5.5).
All had abnormal gait: 62.1% crawled; 37.9% walked on the dorsum. Deform-
ities included equinus, cavus, varus, and adduction, with rigid hypoplastic feet
being common. Main reasons for neglect were multi-factorial, with poor aware-
ness (82.8%), socioeconomic barriers (79.3%), insecurity (62.8%), and limited
access to care (72.4%). All had initial but failed non-operative care and under-
went surgery and by 6 months, all walked erect largely with support; by 9 months,
all walked independently. Subarachnoid block was accepted by 67% (≥8 years).
Challenges included financial constraints (84.5%), long travel distance (63.8%),
insecurity (69.0%) and low parental education (70.7%). Complications were
infrequent and manageable. Conclusion: Tailored surgical management of
NCTEV yields excellent functional and psychosocial outcomes. Age influences
deformity severity and shared decision-making. Multi-factorial neglect neces-
sitates early case finding, public education, and strengthened primary orthopae-
dic services to reduce NCTEV
