EVIDENCE-BASED ANSWER

Emollient cream may alleviate pain and dryness and improve the appearance of heel cracks (strength of recommendation [SOR]: B, one small randomized trial).

Foot soaks followed by mechanical debridement and topical petrolatum may decrease the depth of cracks and thickness of calluses in patients with leprosy (SOR: C, 1 small cohort study).

Keratolytic agents, such as salicylic acid, may reduce hyperkeratosis, cracks, and pain (SOR: C, one case-control study).

Cyanoacrylate tissue adhesives, such as Superglue or Krazy Glue, may reduce pain and speed closure of heel cracks (SOR: C, one case series). Maintenance therapy with emollients and appropriate footwear also may help heel cracks (SOR: C, expert opinion).

Skin hydration of the heel with fissure in patients with diabetes: a cross-sectional observational study

Purpose: Foot fissure should be prevented in patients with diabetes due to the likelihood of subsequent diabetic ulcer. The purpose of this study was to investigate a cutoff point for skin hydration with fissure and the factors associated with low skin hydration in patients with diabetes.
Subjects and methods: Subjects were patients with diabetes who visited the diabetic foot clinic and were evaluated for skin hydration on the heel between April 2008 and March 2015. Information about fissure, skin hydration, age, sex, autonomic neuropathy, angiopathy, and tinea pedis were collected from the medical charts. Skin hydration on the heel was measured using a moisture checker. Skin hydration was compared between heels with and without fissure, and a cutoff for skin hydration with fissure was determined using receiver operating characteristic analysis. Based on the determined cutoff, factors associated with lower skin hydration were analyzed using logistic regression analysis.
Results: Participants comprised 693 patients. Mean±SD age was 66.8±10.8 years, and 57.0% of subjects were male. The frequency of fissures on the heels was 10.4%. Area under the receiver operating characteristic curve for skin hydration in the presence of fissure was 0.717. Twenty percent was selected as the cutoff point, offering sensitivity of 0.478 and specificity of 0.819. Logistic regression analysis showed correlations between three factors (male sex, tinea pedis, and room temperature) and skin hydration <20.0% (odds ratio [OR] 1.587, 95% CI 1.157–2.178, p=0.004; OR 1.548, 95% CI 1.122–2.135, p=0.005; and OR 0.900, 95% CI 0.823–0.0985, p=0.021, respectively).
Conclusion: To prevent heel fissures, moisturizing care should aim at achieving skin hydration of 20%. If skin hydration is <20%, prevention of fissures may warrant not only specialized moisturizing care but also consideration of treatment for tinea pedis.

Evaluation of the Effect of Aloe vera Gel Compared with Lanolin Oil
in Healing of Foot Crack Disease: A Clinical Trial Study

Background: Foot Fissure is one of the most common diseases of the foot.
Objective: This study was conducted to evaluate the effect of aloe vera gel and lanolin oil on the
healing process of foot fissure in the Saveh city.
Methods: In this interventional study that performed on 94 patients with foot fissure in both feet,
the demographic information questionnaire, dry feet clinical assessment scale and the visual
analogue scale (VAS) were used. The subjects in the first intervention group used the aloe vera gel,
while the second intervention group used lanolin. The samples of the control group received placebo
matched with the original medicine. Finally, the data were analyzed using statistical SPSS software,
Ver. 23.
Results: The analysis of data's indicated no significant differences between the three groups before
the intervention regarding the feet dryness. But, two weeks after the intervention, the mean scores of
dry feet in the intervention I group (Aloe vera) was obtained significantly lower than in the
intervention II group (lanolin) (P = 0.027). Also, the scores from visual analogue scale of the foot
dryness showed no significant differences in the three groups before the intervention, while after the
intervention, the mean visual analog score of the intervention I group was significantly (P = 0.007)
lower than in the intervention II group.
Conclusion: The results of this study show a more favorable effect of aloe vera in comparison with
lanolin in healing of dryness and foot cracking.

Heel fissures are a common foot condition that causes discomfort, pain and may lead to lower confidence levels. Hence people suffering from heel fissures generally seek some therapy for relief. Several methods of treatment are available for cracked heels. In this study, we have applied a traditional method of treating heel fissures, by soaking the feet in a warm water bath along with a few ingredients to smooth the skin. Thermal images of the heels were acquired and analyzed to determine whether there is a significant difference in the temperature of the heel fissures before and after therapy. Further, we applied image processing techniques for feature extractions and analysis. It was observed that there is a 2.2% decrease in the average temperature of the left heel, whereas in the right heel there is a 2.6% decrease in the average temperature. Hence, thermal imaging was used as a potential tool in the diagnosis of cracked heels and was found to be applicable for evaluation of the heel therapy process.

anti-bacterial, anti- viral and ant- fungal properties that don’t seem to be solely used
for removing microorganisms and germs from the body however additionally treats
wounds and improves healing. The paste of Haritaki may be a preparation that has
been delineated an associate external remedy for cracked feet. The present study
aims to assess the effectiveness of Haritaki (Terminalia chebula) paste to reduce the
clinical symptoms among adults with cracked feet. The experimental research design
was used to conduct the study in Mangadu village, Kanchipuram district. 30 samples
were selected by using a simple random sampling technique and data collected for
21 days. The demographic data were collected and assessment of efficacy was also
assessed by using specially prepared grading scale for clinical symptoms of cracked
feet and After 21 days, the clinical symptoms were re-assessed. The present study
shows that the demographic variable Religion, Marital status had shown statistically
significant association with post-level of clinical symptoms among adults with
cracked feet at p<0.50 level and the other demographic variables had not shown
statistically significant association with post-level of clinical symptoms among
adults with cracked feet. From the result of this study, it was concluded that the
adults with cracked feet had a reduction in the clinical symptoms grade after
applying Haritaki (Terminalia chebula) paste.
